FAITH FORMATION REGISTRATION 2008-2009


 

   Filling out this form will register your child/children for faith formation classes.  You need to send your payment into the Faith Formation Office by June 30 to receive the $15 early bird discount. 

In any case, payment must be received prior to the first class. 

Registrations received after August 1, 2008 will be for home school only.

 

We will be mailing out class schedules in August.
